Research Project Description

The "Social Determinants of Health Screening Implementation: Bridging Clinic and Community to Improve Healthcare Outcomes" project is intended to investigate: (1) the current design and implementation of social determinants of health (SDOH) screening tools for patients at Parkview Health System; (2) the impacts of SDOH screening on clinician roles, workflows, and patient experiences; and (3) alternative designs and implementation strategies for SDOH screening. The study involves observations, interviews, review of tools currently in use, analysis of existing clinical data and usablity tests. The study will result in recommendations for Parkview health system, and a prototype screening tool.
